Practice at: Willow Creek Community Health Center – Open Door Community Health Centers
The Primary Care Provider (Physician, Nurse Practitioner, or Physician Assistant) delivers comprehensive, patient-centered outpatient care for a panel of patients across the lifespan in a rural Federally Qualified Health Center (FQHC) setting. Practicing to the full extent of their education, training, licensure, and scope of practice, the provider assesses, diagnoses, treats, and manages acute and chronic health conditions; performs appropriate diagnostic and therapeutic procedures; emphasizes preventive care and health promotion; and coordinates specialty services when needed. Working collaboratively within an interdisciplinary care team, the provider plays a vital role in improving access to high-quality healthcare for an underserved rural community, ensuring patients receive timely, comprehensive care close to home while helping reduce the need for travel outside the region for essential medical services.
Willow Creek, CA
Located along the Trinity River, this small, close-knit mountain community offers a unique setting where providers often care for the same families over time. In this environment, providers practice collaborative, relationship-centered care within a team-based model, where access challenges and distance make care more personal—and more impactful. Continuity, trust, and community connection are central to care delivery, allowing providers to see the direct impact of their work on patients and families.
