This full-time role provides mental health and substance use disorder services within an integrated primary care and Medication-Assisted Treatment (MAT) environment. The clinician will work closely with medical providers, behavioral health staff, and community partners to support patients with co-occurring mental health and SUD needs. Responsibilities

  • Provide integrated behavioral health services embedded within the MAT workflow.
  • Conduct SUD and mental health assessments, including SBIRT screening.

  • Facilitate 8-10 week SUD group treatment cycles.
  • Support MAT engagement, adherence, and relapse-prevention strategies.
  • Reduce no-show rates through engagements, reminders, and addressing treatment barriers.
  • Provide crisis intervention, care coordination, and warm handoffs to internal and external partners.
  • Participate in referral-tracking, follow-up, and quality improvement activities.

Qualifications

  • Master's Degree in Social Work, Counseling, or related behavioral health field.
  • Active Michigan licensure: LMSW - Clinical, LPC, LPCC, LLP, or equivalent.
  • Experience working with SUD, MAT, or co-occurring disorders preferred.
  • Training in MI, CBT, SBIRT, or willingness to obtain training.
  • Comfort working in fast-paced, team-based primary care settings.
  • Strong communication, engagement, and crisis-management skills.
