Overview
Creative Health Solutions is an in-house therapy company providing rehabilitation services within the skilled nursing communities we serve. We are looking for a PRN Physical Therapist to extend our reach through telehealth. Position SummaryWe are seeking a licensed Physical Therapist for a consistent PRN telehealth position covering four Florida skilled nursing facilities. These communities include skilled nursing, assisted living, and memory care residents. You will work remotely with on-site clinical staff to keep residents progressing safely toward their goals. All telehealth visits are limited to evaluations, recertifications, or supervisory visits. Every telehealth visit is conducted with a licensed PTA or COTA at the facility. Required- Active, unrestricted Florida Physical Therapy license
- Residence within one hour of one of our facilities in Wesley Chapel, Brooksville, Winter Haven, or Ocala, FL
- Reliable high-speed internet and a private, quiet workspace for telehealth
- Strong communication skills and comfort with technology

- Develop and administer therapy programs aimed at improving patients' physical, mental, and emotional well-being.
- Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
- Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
- Continuously educate patients and their families on therapeutic procedures and techniques
- Stay up to date with the latest therapy techniques and methodologies.
