At CareMore Health, our Institutional Special Needs Plan (ISNP) physicians care for medically complex seniors living in skilled nursing facilities and long-term care settings. These patients often face multiple chronic conditions, frequent transitions of care, and significant health challenges that require thoughtful, coordinated medical leadership.

As an ISNP Physician, you'll have the opportunity to build lasting relationships with patients, families, facility partners, and interdisciplinary care teams while improving health outcomes for some of the most vulnerable members of our communities.

How You'll Make an Impact

  • Provide comprehensive primary care services to ISNP members in SNF and LTC environments.
  • Perform admission assessments, routine visits, and acute evaluations.
  • Develop individualized care plans for medically complex patients.
  • Lead transitions of care following hospitalizations.
  • Support advanced care planning and patient education.
  • Drive quality measures and population health outcomes.

Qualifications

  • MD or DO from an accredited medical school
  • Active Arizona medical license
  • DEA registration
  • Board Certified or Board Eligible
  • Experience with geriatrics, post-acute care, SNF, LTC, or complex patient populations preferred

Compensation Range:

$211,369.00

to

$317,053.00

 

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
