Location: Cadillac, MI

Description:

 The Patient Navigator is an integrated member of the Patient Centered Medical Home team providing direct support and assistance in the coordination of medical services under the direction of clinical leadership, standing orders, policies and procedures.

Duties:

Serves as an advocate for the patient’s medical needs.  Serves as the patient’s primary contact for questions and communications with the health care team, assuring both timeliness and efficiency.

Requirements include the following:

This person must be a certified Medical Assistant or at least one year experience as a medical assistant and have excellent phone and customer service skills.
